Position Summary: Reporting to the Director of Quality Improvement. Responsible for interacting on a professional basis with members of the medical staff, administration, management, patient care services and ancillary departments. Ability to maintain positive customer relations.
Graduation from an accredited school of nursing
Current registration in the state of practice.
3 years clinical experience
1 year case management experience or similar role preferred
C. Special qualifications:
Excellent oral and written communication skills
Excellent interpersonal skills with groups or one to one
Knowledge of medical terminology, levels of care, treatment modalities
Ability to make competent professional judgments and decisions
Understanding of the formal and informal organizational structure
Demonstrate a desire and willingness to maintain and upgrade professional skills and education
D. Outside Relationships:
Maintain a positive relationship with community agencies
Active in professional organizations
Integrate with various members of the health care organization for transitions of care and readmission prevention.
Able to pull and analyze data for various quality project across the continuum of care.
Assist senior data analysist and director to retrieve data from various sources and Participates in department & facility QA & Performance initiatives.
Follows FOCUS-PDCA process.
5. Actively participates in meetings, committees, and continuing education to improve organizational, departmental, and individual performance.
6. Collects and analyzes data and information to enhance and improve the department, hospital and organization operations.
7. Follows Established P&P to identify and reduce the risks of acquiring or transmitting infection.
Identifies and facilitates team improvement in the quality, completeness and accuracy of clinical documentation.
Demonstrates knowledge of Core Measures’ documentation requirements.
Demonstrates clinical expertise skills and behavior appropriate to the population(s)served.
Obtains appropriate clinical documentation through extensive interaction with physicians, Patient Care Director and staff
Improves overall quality and completeness of clinical documentation by performing admission/continued stay reviews.
Reviews clinical issues with physician advisor to address completeness of documentation.
Maintains open communication with Case Managers, Physicians to ensure quality of patient care, as appropriate, refers instances of inappropriate patient care, discharge delays, etc. to appropriate person.
Promotes the use of external benchmarking/best practices and comparative data.
Conducts follow-up reviews of clinical documentation to ensure issues discussed and clarified with the physician have been recorded in the patient’s chart.
Maintains positive open communication with physicians, inter-disciplinary care team members and Department Director.
Assists in special projects as needed.
